Purchase Request Form

The Consortium Library welcomes requests from registered patrons in the UAA and APU communities for monographic materials which support research, study, and teaching at the universities.

Before submitting your request, search the Library Catalog to ensure your title is not already in the library's collection.

Important Information

• Please complete the form as accurately as possible and note that the starred (**) fields are required.
• An incomplete form will not be considered for purchase.
• The electronic method of request is not recommended for multiple (five or more) title requests.
• Please continue to submit multiple requests to Pauletta Sawyer in Collection Development in the usual manner, i.e., catalog pages, website print-outs, publisher advertisements, brochures, etc.
