 APU students, faculty, and staff are pre-activated and do not need to fill out this form, just present your APU ID Card when borrowing materials.

If your APU ID Card does not have a library card number printed on the back, contact the APU IT Helpdesk at ithelpdesk@alaskapacific.edu or 564-8350.
