UAA students are pre-activated and do not need to fill out this form. Only UAA faculty and staff should fill this form out.
After activation, your UAA Wolfcard is your library card. You must present your Wolfcard when borrowing materials from the Library.
Please note that activation requires 24 hours.
Library cards expire in September and may be renewed at that time.
